ADHD Testing Information

  • Initial Appointment Information

    Provider: Taylour Kidd, LPCC-S
    Purpose: To gather necessary background information discuss your concerns, and determine if ADHD testing is appropriate
    Billing Options:
    Insurance: The initial intake session can be billed thorugh insurance (check your benefits for verification.)
    Self-Pay: If insurance is not used, the initial fee is $150.00.

  • ADHD Testing Process

    Our ADHD evaluation includes comprehensive components to ensure accurate assessment:

    • Separate clinical interview (outside of the intake)
    •  Administration of tests
    •  Interpretation of results
    •  Consultation, when needed
    •  Comprehensive written report
    •  Appointment to review results and recommendations

    Important: ADHD testing is self-pay only and will not be billed through insurance.

  • Appointment and Testing Details

    • Cost: $130.00 per hour
    •  Location: 755 White Pond Drive, Akron, Ohio 44320
    • In-Office: Testing administration is carried out in our office setting.
    • Virtual/ In-Office: All other appointments, including the clinical interview and
      consultation, are available virtually or in-office.

  • Payment Policy

    •  The full balance must be paid in advance of the appointment dedicated to reviewing
      results and recommendations.
    • A superbill is available upon request for potential out-of-network reimbursement
      submissions.

  • For Ages 8-17

    • Vanderbilt to determine necessity for further evaluation
    • Completed by a guardian and a teacher
    • Detailed history, symptoms, and impairments
    • Observations in session and during assessment

  • For Ages 18+

    •  Adult ADHD Self-Report Scales (ASRS-v1.1) Symptom Checklist to determine necessity for further evaluation
    • Detailed history, symptoms, and impairments
    • Observations in session and during assessment

Relevant Testing for Ages 18+

Testing information

  • Conners CPT 3 – assess performance in areas of inattentiveness, impulsivity, sustained attention, and vigilance
  • Symptom Checklist-90-R (SCL-90-R) – assess likelihood of having a clinically significant problem in 9 areas including somatization, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation, and psychoticism
  • Brown Executive Function/Attention Self Report Scales (Brown EF/A) – assess self report of issues pertaining to executive functioning in 6 areas including activation, focus, effort, emotion, memory, and action.
  • Any additional measures determined appropriate by clinician
